Description - BORTEZOMIB Zentiva dry sub 3.5 mg
Bortezomib Zentiva
Indikation
Multiples Myelom: ohne Vorbehandlung in Kombination mit Melphalan/Prednison, in Kombination mit Standardchemotherapie und Stammzelltransplantation, rezidivierend/refraktär nach min. einer Vorbehandlung.Mantelzell-Lymphom: wenn eine Stammzelltransplantation nicht durchführbar ist: ohne Vorbehandlung in Kombination mit Rituximab, Cyclophosphamid, Doxorubicin und Prednison, rezidivierend/refraktär im Stadium III/IV mit Translokation t(11; 14) (q13; q32) oder Überexpression von Cyclin D1 nach min. einer Vorbehandlung.
Dosierung
Bolus i.v. über 3?5 Sek. (nach Rekonstitution mit NaCl 0,9% auf eine Konz. von 1 mg/ml) oder s.c. (nur Bortezomib 3,5 mg nach Rekonstitution mit 1,4 ml NaCl 0,9%; Konz.: 2,5 mg/ml).Multiples Myelom
Kombination mit Melphalan/Prednison: (p.o.: Melphalan 9 mg/m2 und Prednison 60 mg/m2 am Tag 1?4 jedes 2. Zyklus): >18 J.: erste 8 Zyklen: 1,3 mg/m2 am Tag 1/4/8/11, dann 10 Tage Pause, Zyklen 9?18: 1,3 mg/m2 am Tag 1/8.
Kombination mit Standardchemotherapie/Stammzelltransplantation: >18 J.: 1,3 mg/m2 am Tag 1/4/8/11, dann 10?18 Tage Pause, während 3?4 Zyklen.
Rezidivierend/refraktär: >18 J.: 1,3 mg/m2 am Tag 1/4/8/11, dann 10 Tage Pause, nach vollständigem Ansprechen: 2 zusätzliche Zyklen, bei unvollständiger Remission: insgesamt 8 Zyklen.
Mantelzell-Lymphom
Kombination mit Rituximab, Cyclophosphamid, Doxorubicin, Prednison: (i.v. Inf.: Rituximab 375 mg/m2, Cyclophosphamid 750 mg/m2 und Doxorubicin 50 mg/m2 am Tag 1 jedes Zyklus, p.o.: Prednison 100 mg/m2 am Tag 1?5 jedes Zyklus): >18 J.: 1,3 mg/m2 am Tag 1/4/8/11, dann 10 Tage Pause; Therapiedauer: 6 Zyklen, bei erstmaligem Ansprechen in Zyklus 6: 2 zusätzliche Zyklen.
Rezidivierend/refraktär: >18 J.: 1,3 mg/m2 am Tag 1/4/8/11, dann 10 Tage Pause, nach vollständigem Ansprechen: 2 zusätzliche Zyklen, bei unvollständiger Remission: insgesamt 8 Zyklen.
Kontraindikation
Schwangerschaft, Stillzeit.
