When should Lyman 200'000 forte Emgel, gel or ointment not be used?
In the event of hypersensitivity to one of the active ingredients or one of the excipients, Lyman 200'000 forte should not be used Emgel, gel or ointment are not used. In the case of heparin-induced thrombocytopenia (HIT, lack of blood platelets caused by heparin), Lyman 200'000 forte Emgel, gel or ointment must not be used. Lyman 200'000 ointment contains peanut oil and should not be used if you are hypersensitive to peanuts or soya.
When should you be careful when using Lyman 200'000 forte Emgel, gel or ointment?
Lyman 200'000 forte Emgel contains 10 mg benzyl alcohol per 1 g Emgel and macrogolglycerol hydroxystearate. Benzyl alcohol may cause allergic and mild local irritation. Macrogolglycerol hydroxystearate may cause skin irritation. Lyman 200'000 forte Gel contains 10 mg benzyl alcohol per 1 g gel. Benzyl alcohol may cause allergic and mild local irritation. Lyman 200'000 forte ointment contains peanut oil, 150 mg propylene glycol, cetostearyl alcohol, 1 mg - 5 mg sodium lauryl sulfate, methyl parahydroxybenzoate and propyl parahydroxybenzoate per 1 g of ointment. The ointment contains peanut oil. It must not be used if you are hypersensitive (allergic) to peanut or soy. Propylene glycol may cause skin irritation. Cetylstearyl alcohol may cause localized skin irritation (e.g. contact dermatitis). Sodium Lauryl Sulfate may cause local skin reactions (like a stinging or burning sensation) or increase skin reactions caused by other products applied to the same skin area. Methyl parahydroxybenzoate and propyl parahydroxybenzoate can cause allergic reactions, including delayed reactions. Wash your hands after applying or rubbing in Lyman 200'000 forte Emgel, gel or ointment. Avoid contact with eyes. Use only on intact skin, not on mucous membranes and on open wounds. In the case of venous diseases that are due to the presence of a blood clot (a so-called thrombosis), massage should not be used. If the symptoms persist or worsen, a doctor should be consulted. Due to the heparin contained in Lyman 200'000 forte Emgel, gel or ointment, an interaction with medicines that inhibit blood clotting (these include medicines for blood thinning as well as many painkillers and rheumatism medicines) cannot be completely ruled out. How do you use Lyman 200'000 forte emgel, gel or ointment?
Adults: Unless otherwise instructed by the doctor prescribed, apply an approx. 5 cm long strand to the areas of intact skin to be treated and to the surrounding areas of skin 2-3 times a day and rub in lightly. Massage direction for the legs: from bottom to top. In the case of phlebitis, do not rub in the Emgel, gel or ointment, but apply it thickly with a knife and put on a bandage. This type of application is not suitable for children, since an increased absorption of the active ingredient through the skin cannot be ruled out, especially in small children due to the sealing effect of the bandage. The duration of use is not limited, but if the symptoms persist or worsen, the doctor should be consulted. Children and adolescents: The use and safety of Lyman 200'000 forte Emgel, gel or ointment in children and adolescents has not yet been tested. Lyman 200'000 Gel has a cooling effect and is suitable for treating large areas that are sensitive to pain. When Lyman 200'000 forte ointment is massaged in, a white film initially forms on the skin, which disappears as the massage continues, as the ointment penetrates completely into the skin. Indikationen/Anwendungsmöglichkeiten
•Bei Beschwerden im Zusammenhang mit Krampfadern wie Schmerzen, Schweregefühl und geschwollenen Beinen (Stauungsödeme).
•Bei stumpfen Sport- und Unfallverletzungen wie Prellungen, Quetschungen, Schwellungen, Zerrungen und Blutergüssen.
•Bei Schmerzen der Muskeln und Sehnen.
Auf ärztliche Verschreibung kann Lyman 200'000 forte Emgel, Gel bzw. Salbe auch bei (oberflächlichen) Venenentzündungen, zur Nachbehandlung von Verödungen sowie unterstützend bei Venenthrombosen angewendet werden.
Dosierung/Anwendung
Erwachsene: 2–3 mal täglich einen ca. 5 cm langen Strang auf die zu behandelnde Stelle sowie die umgebenden Hautpartien auftragen und leicht einreiben. Nur auf intakter Haut und nicht auf Schleimhäuten anwenden. Massagerichtung bei den Beinen: von unten nach oben.
Bei Venenentzündungen nicht einreiben, sondern messerrückendick auftragen und einen Verband anlegen.
Beim Einmassieren von Lyman 200'000 forte Salbe bildet sich zunächst ein weisser Film auf der Haut, der bei weiterem Massieren verschwindet, da die Salbe völlig in die Haut eindringt.
Kinder und Jugendliche: Die Anwendung und Sicherheit von Lyman 200'000 forte Emgel, Gel bzw. Salbe bei Kindern und Jugendlichen ist bisher nicht geprüft worden.
Kontraindikationen
Bei einer Überempfindlichkeit gegenüber einem der Wirkstoffe oder einem der Hilfsstoffe gemäss Zusammensetzung und bei Heparin-induzierter Thrombozytopenie (HIT).
Lyman 200'000 forte Salbe enthält Erdnussöl und darf bei einer Überempfindlichkeit gegenüber Erdnuss oder Soja nicht angewendet werden.
Warnhinweise und Vorsichtsmassnahmen
Lyman 200'000 forte Emgel enthält 10 mg Benzylalkohol und 3.6 mg Macrogolglycerol hydroxystearat pro 1 g Emgel. Benzylalkohol kann allergische und leichte lokale Reizungen hervorrufen. Macrogolglycerol hydroxystearat kann Hautreizungen hervorrufen.
Lyman 200'000 forte Gel enthält 10 mg Benzylalkohol pro 1 g Emgel. Benzylalkohol kann allergische und leichte lokale Reizungen hervorrufen.
Lyman 200'000 forte Salbe enthält Erdnussöl, 150 mg Propylenglykol, Cetylstearylalkohol, 1 – 5 mg Natriumdodecylsulfat, 2.8 mg Methylparahydroxybenzoat und 1.2 mg Propylparahydroxybenzoat pro 1 g Salbe.
Lyman Salbe darf nicht angewendet werden, wenn eine Allergie gegenüber Erdnuss oder Soja besteht. Propylenglycol kann Hautreizungen hervorrufen. Cetylstearylalkohol kann örtlich begrenzt Hautreizungen (z.B. Kontaktdermatitis) hervorrufen. Natriumdodecylsulfat kann lokale Hautreaktionen (wie ein stechendes oder brennendes Gefühl) hervorrufen oder Hautreaktionen verstärken, die durch andere auf dieselbe Hautstelle aufgebrachte Produkte verursacht werden. Methylparahydroxybenzoat und Propylparahydroxybenzoat können allergische Reaktionen, auch Spätreaktionen, hervorrufen.
Lyman 200'000 forte Emgel, Gel bzw. Salbe darf nur auf intakter Haut, nicht auf offene Wunden und/oder bei eitrigen Prozessen nicht auf die Infektionsstelle, appliziert werden. Der Kontakt mit Schleimhäuten und der Augenbindehaut ist zu vermeiden. Bei thrombotischen und thromboembolischen Prozessen darf nicht einmassiert werden.
Bei Verschlechterung der Symptome ist ein Arzt bzw. eine Ärztin aufzusuchen.
Interaktionen
Es liegen keine Hinweise aus Studien oder von Kasuistiken vor, die einen Interaktionsverdacht zwischen topisch appliziertem Heparin und systemisch wirkenden Gerinnungshemmern begründen würden. Für systemisch verfügbares Heparin besteht die Möglichkeit der zusätzlichen Steigerung des Blutungsrisikos bei gleichzeitiger Behandlung mit Antikoagulantien (z.B. Cumarine, Acetylsalicylsäure, Dipyridamol, Dextrane, Streptokinase). Es könnte zu einer Kumulation der Wirkung kommen.
Schwangerschaft/Stillzeit
Es liegen keine klinischen Daten mit Anwendung bei Schwangeren vor. Es liegen keine hinreichenden tierexperimentellen Studien zur Auswirkung auf Schwangerschaft, Embryonalentwicklung, Entwicklung des Fötus und die postnatale Entwicklung vor. Das potentielle Risiko für den Menschen ist nicht bekannt.
Bei der Anwendung während der Schwangerschaft ist Vorsicht geboten.
Wirkung auf die Fahrtüchtigkeit und auf das Bedienen von Maschinen
Es wurden keine entsprechenden Studien durchgeführt. Da der Wirkstoff Heparin nur zu einem geringen Anteil systemisch verfügbar ist, ist jedoch mit keiner Beeinträchtigung der Fahrtüchtigkeit oder der Maschinenbedienung zu rechnen.

Lyman 200'000 forte Emgel, Gel bzw. Salbe ist eine Kombination aus den Wirkstoffen Heparin, Dexpanthenol und Allantoin.
Wirkungsmechanismus
Heparin zeichnet sich durch eine antithrombotische, entzündungshemmende, schmerzstillende und resorptionsfördernde Wirkung aus. Die transepitheliale Durchschleusung von Heparin wird durch die gemeinsame Wirkung der beiden Komponenten Dexpanthenol und Allantoin beschleunigt.
Dexpanthenol unterstützt die entzündungshemmenden Eigenschaften des Heparins und bewirkt durch seinen granulations- und epithelisationsfördernden Effekt einen Schutz der Haut.
Allantoin stimuliert die Stoffwechselvorgänge der unter der Haut liegenden Bindegewebsschichten, indem das Zellwachstum sowie der Abtransport von Stoffwechselprodukten angeregt werden. Darüber hinaus trägt Allantoin durch seine keratolytischen Eigenschaften und die daraus folgende Lockerung der sonst wenig durchlässigen Lederhaut zu einer Verbesserung der Heparin-Resorption bei.

Absorption
Heparin durchdringt nach äusserlicher Anwendung die Hornschicht und ist in der Epidermis und im Corium nachweisbar. Die Aufnahme von Heparin in den Körper nach äusserlicher Anwendung hängt hauptsächlich von der Fläche und der Häufigkeit der Anwendung ab. Sie ist bei korrekter Anwendung zu gering, um eine systemische Wirkung zu entfalten.
Distribution
Dexpanthenol besitzt Vitamin B-Charakter und ist als Vorstufe von Coenzym A essentiell. Es wird in der Haut in das Vitamin Pantothensäure umgewandelt, welche im Zellgewebe rasch verteilt wird und für die normale Funktion der Gewebe sorgt, insbesondere der epithelialen Schichten.
